Attention Dorado Athletes:

For Athletic participation it is YOUR responsiblity to:

  1.  Complete the Athletic packet completley and accurately with Parent/Guardian signatures. 

  2. Schedule and complete the Physical exam and Health Questionnaire with Signatures from doctor on both pages.

  3. Complete the Brainbook AND OPIOID COURSE online at https://academy.azpreps365.com/  and attach the certificate of completion with your athletic packet.

  4. Pay the $76.00 Athletic Participation fee at the bookstore. The receipt must be attached to your packet.

  5. Completed packets turned  BEFORE 12:00pm will be cleared to start the same day. If your (completed) packet is turned in AFTER 12:00pm you will have to wait until the next day to be cleared to start.
